I think I always left my tips the last night of a long stay, which is kind of stupid when you think about it. But I was under the impression they were to be left on the pillow, not that I'd leave a $100 bill lying on a table and expect to see it again.
 
A lot is expected out of the housekeepers to "thoroughly" clean every room on their list. I used to work at a hotel and yes, the rooms are pretty nasty after knowing what goes on.

But if you are staying in a room, the first thing you should do is toss the covers aside and avoid touching it if possible. Bed covers rarely get washed and who knows how long it has been before the previous time it got washed/who else was on it, and what was done on it.
Also don't use the coffee pot/cups also. Housekeepers are suppose to have different spray cleaners for different things. ie. the blue spray for windows/mirrors, another for the toilet/tub/floors etc.
Yes, sometimes they'll run out or are just lazy and use the same spray to wash all of it.

Also you don't want to walk around bare foot on the carpet, wear sandals/slippers if not atleast socks.

If you are staying a few nights, I recommend that you do put the DND sign outside the doors just to let them know not to disturb you. They sometime do come by pretty early if you want to sleep in, or if you don't want someone walking around with all your personal belongings while you're out. You can always call the front desk to ask the housekeepers to refresh/take out the towels and if you want new sheets.
