Well, for starters, the Lakers have at least 4 alternate jerseys that they've not worn in games. I think the Sixers have a blue one that I've never seen them wear. The Knicks have one that is red, white and blue instead of orange,white and blue. I'm sure there are more that I've missed.

Well there's a huge difference in NBA team issued alternate jerseys and the jerseys you are talking about. The Sixers do wear their blue jerseys. Most of the jerseys you are talking about though are not team issued alternate jerseys, rather a novelty sold in stores to make more money. I am talking about legitimate team issued alternate jerseys. The Heat have their 3, the Lakers have their 3, the Sixers and so on. The Cavs also have a alternate jersey that is black but i believe the rule says you can't have an alternate until you have had your "new" jerseys for three years. I have not heard that the black Kings jersey was supposed to be an alternate but i wish they'd make it one.
